TIMI Risk Score (STEMI)

Calculate the TIMI Risk Score for ST-elevation MI to predict 30-day mortality.

Formula

Sum of 8 criteria. Score range 0–14; each point increases 30-day mortality.
  • Higher TIMI-STEMI score = higher 30-day mortality.
  • Used for risk communication and triage in STEMI.
  • Reference: Morrow DA et al. Circulation 2000;102:2031.

Low-risk STEMI

Inputs
  • Age 65–74 (2 pts) or ≥75 (3 pts): 0
  • DM, HTN, or angina: 1
  • Systolic BP < 100 mmHg: 0
  • Heart Rate > 100 bpm: 0
  • Killip II–IV: 0
  • Weight < 67 kg: 0
  • Anterior ST elevation or LBBB: 1
  • Time to treatment > 4 hours: 0

DM(1)+Anterior(1)=2. ~2.2% 30-day mortality.

Frequently asked questions

How does TIMI-STEMI guide treatment?
TIMI-STEMI helps communicate risk to patients and families. It does not directly change management (all STEMI patients need reperfusion) but helps guide intensity of monitoring and adjunct therapies.
