Formula

Sum of 6 organ systems (0–4 each); max = 24
  • Sepsis-3 definition: SOFA score ≥2 from baseline in suspected infection.
  • Delta-SOFA (change from baseline) is more predictive than absolute score.
  • ICU mortality: score 0–6 <10%, 7–9 ~20%, 10–12 ~40%, 13–14 ~50%, 15+ >80%.

Per Sepsis-3 (Singer et al. JAMA 2016), sepsis is defined as life-threatening organ dysfunction (SOFA ≥2) caused by a dysregulated host response to infection. Septic shock is SOFA ≥2 + vasopressors needed + lactate >2 mmol/L.
